Quest Insurance Provider List

This list is not exhaustive and may or may not include all of the health insurance companies with which Dermpath Diagnostics is in-network. There may be exceptions in certain areas or for certain members or plans. For example, certain self-funded plans may have a custom provider network. Information is believed to be accurate as of the production date; however, it is subject to change. Even when in-network, not all testing performed by Dermpath may be covered by a member’s plan. To confirm eligibility for any specific benefits under a health plan, call the Member Services telephone number on the member’s ID card.
*For out-of-area BlueCross BlueShield plan members, network status is generally determined by the ordering physician’s zip code and Dermpath contract with the Blues plan in that service area. Patients should call the Member Services telephone number on their member ID card to verify their out-of-area benefits. In Pennsylvania, in Bucks, Chester, Delaware, Montgomery, and Philadelphia counties, claims may be treated as out-of-network. When selecting “location” on the site, users should select the ordering physician’s state, rather than the lab collection state, if these locations differ.
