Network status is based on the contract between the health insurance company and the above respective company. In the following referenced states, laboratory services may be provided by: Mid America Clinical Laboratories (Indiana), Associated Clinical Laboratories (Pennsylvania), and Quest Diagnostics Venture (Pennsylvania). When applicable, network status is based on the contract between the health insurance company and the above respective company.
*For out-of-area BlueCross BlueShield plan members, network status is generally determined by the ordering physician’s zip code and Quest’s contract with the Blues plan in that service area. Patients should call the Member Services telephone number on their member ID card to verify their out-of-area benefits. In Pennsylvania, in Bucks, Chester, Delaware, Montgomery, and Philadelphia counties, claims may be treated as out-of-network. When selecting “location” on the site, users should select the ordering physician’s state, rather than the lab collection state, if these locations differ.
